On assignment in Budapest to intercept a courier working for a person of interest code-named "Cobalt", IMF agent Trevor Hanaway is killed by assassin Sabine Moreau. Hanaway's team leader, Jane Carter, and newly promoted field agent Benji Dunn extract Ethan Hunt and Hunt's source Bogdan from a Moscow prison. Hunt is recruited to lead Carter and Dunn to infiltrate the secret Moscow Kremlin archives and locate files identifying Cobalt. During the mission, someone broadcasts across the IMF frequency, alerting the Russians to Hunt's team. Although Dunn and Carter escape, a bomb destroys the Kremlin, and Russian agent Sidorov arrests Hunt, suspecting him as part of the attack.
The IMF extracts Hunt from Moscow. The Russians have called the attack an undeclared act of war, and the US president activates "Ghost Protocol", a black operation contingency that disavows the entire IMF. Hunt and his team are to take the blame for the attack, but they will be allowed to escape from government custody so that they may track down Cobalt. Before Hunt can escape, the IMF's secretary is killed by Russian security forces led by Sidorov, leaving Hunt and intelligence analyst William Brandt to find their own way out. The team identifies Cobalt as Kurt Hendricks, a Swedish-born Russian nuclear strategist who believes the weak must die for the strong to survive, so he plans to start a nuclear war to initiate the next stage of human evolution. Hendricks bombed the Kremlin in order to acquire a Russian nuclear launch-control device; however, he now needs the activation codes from the Budapest courier in order to launch a nuclear missile at America.

In Bogota, Colombia, Fabio Maria Restrepo (Jesse Borrego), a gangster and drug runner, is having a meeting with Don Luis (Beto Benites), a drug lord, to pay off his remaining debt and leave his employment. Fabio, however, knows that Don Luis will put a hit out on him for leaving with a valuable item that he holds and Don Luis wants - a small disk with valuable information on the drug lords - so he rushes home to prepare his wife, Alicia (Cynthia Addai-Robinson) and his young daughter, Cataleya (Amandla Stenberg).While Alicia packs, Fabio hands Cataleya a business card, which should give her entry to the U.S. embassy, in case she finds herself alone. Fabio hands Cataleya the item that Don Luis wants, telling her not to lose it and to give it to the Ambassador telling her that it is "her passport". He also gives Cataleya an address in Chicago. Alicia finishes packing, and Fabio grabs his wife and child and prepares to escape; at that point, however, Marco and his thugs have the house surrounded. Fabio and Alicia have Cataleya sit at the kitchen table and run down the stairs with guns, attempting to hold off Don Luis's capos. Cataleya watches both her parents being gunned down.One of Marco's thugs runs up the stairs and points a gun at Cataleya, but Marco waves him away and sits across the table with her. He tells her that her father died because he betrayed Don Luis, but tells Cataleya that "smart girls always get what they want," and instructs her to give him the disk that her father handed her. Cataleya instead stabs Marcos in the hand with a kitchen knife and escapes her house. She runs through the streets of Bogota, pursued by thugs from all directions, and is able to slip away by hiding inside a water sewer. She then uses the business card her father gave her to get into the U.S. Embassy and goes to the Ambassador's office. Cataleya regurgitates the disk which she swallowed onto the Ambassador's desk. In exchange for this, they give her a passport and a small amount of cash, and fly her to Miami.

In 1891, Irene Adler (Rachel McAdams) delivers a package to Dr. Hoffmanstahl—payment for a letter he was to deliver. Hoffmanstahl opens the package, triggering a hidden bomb which is prevented from detonating by the intervention of Sherlock Holmes (Robert Downey, Jr.). Holmes takes the letter and disposes of the bomb while Adler and Hoffmanstahl escape. Holmes later finds Hoffmanstahl assassinated. Adler meets with Professor Moriarty (Jared Harris) to explain the events, but Moriarty poisons her—deeming her position compromised by her love for Holmes.
Some time later, Dr. Watson (Jude Law) arrives at 221B Baker Street, where Holmes discloses that he is investigating a series of seemingly unrelated murders, terrorist attacks, and business acquisitions, that he has connected to Moriarty. Holmes meets with the gypsy Simza (Noomi Rapace), the intended recipient of the letter he had taken from Adler, sent by her brother Rene. Holmes defeats an assassin sent to kill Simza, but she flees before Holmes can interrogate her. After Mary (Kelly Reilly) and Watson's wedding, Holmes meets Moriarty for the first time. Moriarty informs Holmes that he murdered Adler and will kill Watson and Mary if Holmes' interference continues.
Moriarty's men attack Watson and Mary on a train to their honeymoon. Holmes, having followed the pair for protection, throws Mary from the train into a river below where she is picked up by Holmes' waiting brother Mycroft (Stephen Fry). After defeating Moriarty's men, Holmes and Watson travel to Paris to locate Simza. When she is found, Holmes tells Simza that she has been targeted because Rene is working for Moriarty, and may have told her about his plans. Simza takes the pair to the headquarters of an anarchist group to which she and Rene had formerly belonged. They learn that the anarchists have been forced to plant bombs for Moriarty.
